Arise and Shine Health Center uses SMS (text messaging) to communicate efficiently with clients and visitors. By providing your phone number and agreeing to receive SMS messages, you consent to these Terms and Conditions. The following terms explain how we manage SMS communication and your rights related to receiving text messages.

  1. Consent to Receive SMS Messages

    By providing your phone number and consenting to receive text messages from us, you authorize Arise and Shine Health Center to send SMS messages to the phone number you provided.

    This consent applies to communications related to:

    • Appointment reminders and confirmations
    • Telehealth session links or instructions
    • Scheduling and follow-up updates
    • Care coordination and treatment reminders
    • Responses to general inquiries or patient requests
    • Important health-related or administrative notifications

    No mobile opt-in, SMS consent, or phone numbers will be shared or sold to any third party or affiliate for marketing or promotional purposes. All information collected through SMS communication is kept confidential and used solely to provide requested healthcare services or respond to direct inquiries. We take appropriate measures to protect your data and ensure it remains secure.

  2. How to Opt In (Opt-In Language)

    When you provide your phone number, you may see a statement such as:

    “By providing your mobile number and checking this box, you consent to receive SMS messages from Arise and Shine Health Center regarding your appointments, telehealth sessions, service updates, responses to your general inquiries, and other important notifications. Message frequency may vary. Reply STOP to unsubscribe or HELP for assistance. Standard message and data rates may apply.”

    You may also verbally confirm your consent with our staff or sign up through an online or paper form.

  3. Types of SMS You May Receive

    If you have consented to receive text messages, you may receive messages such as:

    • Appointment Reminder: “Reminder: Your appointment with Arise and Shine Health Center is scheduled for [date/time]. Reply YES to confirm or call 443-209-7100 / 443-374-5044 to reschedule.”
    • Telehealth Session Link: “Your telehealth session link is now available. Please join your session at your scheduled time.”
    • Care Coordination: “Your provider has sent you follow-up instructions. Please check your patient portal or call our office for details.”
    • General Communication: “Thank you for contacting Arise and Shine Health Center. A team member will reach out shortly.”
  4. Message Frequency and Fees

    The number of messages you receive will depend on your interactions with us and may vary from a few messages per week during active treatment periods.

    Standard text messaging and data rates may apply based on your mobile carrier plan. Arise and Shine Health Center is not responsible for any charges billed to you by your mobile provider.

  5. How to Opt Out

    You can stop receiving text messages from us at any time by:

    • Replying STOP to any message (this will unsubscribe you from all future messages).
    • Contacting us directly at 443-209-7100 or 443-374-5044 and requesting removal from our SMS list.
    • Declining or leaving the SMS consent box unchecked on any forms.

    After opting out, you may receive one final confirmation message stating that you have been unsubscribed.

    Reply HELP to any of our messages for assistance. You will receive a message with further contact information:
